New Community Resource Centers Across L.A. County

L.A. Care and Blue Shield Promise together improve member and community health

Community Resource Centers Help Members Stay Active, Healthy and Informed

We are excited to announce that L.A. Care Health Plan and Blue Shield of California Promise Health Plan are investing a combined $146 million as part of a five-year commitment to expand Community Resource Centers across Los Angeles County.

The two health plans have a long history of working together, as Blue Shield Promise is one of the Plan Partners that L.A. Care contracts with to serve Medi-Cal members in Los Angeles County. This new collaboration will leverage the strengths of the two plans to help improve overall health outcomes for members and the communities in which they live. Improved health outcomes will also reduce health care costs over time.

The Community Resource Centers will connect members from both plans and the Los Angeles Community to classes and personalized services that will foster community connections, address social needs and keep them active, healthy and informed. 

What you need to know:
  • $146 million investment over five years by the mission-driven health plans 
  • The plans will add seven more centers over the next few years, bringing the total to 14. The plans will jointly operate existing Resource Centers located in Boyle Heights, East L.A., Huntington Park, Inglewood, Lynwood, Pacoima and Palmdale 
  • The Resource Centers are open to the entire community
  • The Resource Centers offer:
    • Fitness and Health Education Classes
    • Preventive Health, Trainings and Screenings
    • Nutrition and Healthy Cooking Classes, and a Registered Dietician On Site
    • Social Service Assistance (housing, financial literacy, food insecurity)
    • Support Groups
    • On site Department of Public Social Services Worker
  • The new collaboration will allow for personalized services for members including care management closer to the members they serve, and health advocates who will offer concierge level services to meet individual needs
  • It will also allow for additional health screenings, telehealth and other tech support
  • The Community Resource Center programs ultimately can help improve health outcomes, which can also reduce health care costs over time
  • L.A. Care Health Plan’s existing six Family Resource Centers in Boyle Heights, East L.A., Inglewood, Lynwood, Pacoima and Palmdale will continue to operate and remain open to our members and the community. They will gradually transition into Community Resource Centers by the end of 2020.
