Cal MediConnect allows L.A. Care to manage Medicare and Medi-Cal benefits and services, including medical care, long-term supports and services, behavioral and mental health care services, and social supports.
The coordination helps ensure members get the right care, at the right time, in the right setting which suits their individual needs.
In order to be eligible for the Cal MediConnect Plan you must meet the following criteria:
Live in Los Angeles County (our service area), and
Are 21 years of age or older at the time of enrollment, and
Have both Medicare Part A and Medicare Part B, and
Are currently eligible for full Medi-Cal
Are not covered under an employee benefit plan (or your spouse’s)
Do not have other private or public health insurance
Do not receive services through California’s regional centers or state developmental centers or intermediate care facilities for the developmentally disabled
Do not have ESRD (End Stage Renal Disease) at the time of enrollment
If you reside in a nursing home, are enrolled in the Multipurpose Senior Services Program or have In-Home Supportive Services, you must continue to meet your share of cost, if you have one.
